Immunizations

Indiana requires that all students have specific vaccinations prior to attending school to minimize student exposure to preventable, life-threatening disease. If your child is behind on their immunizations, the health center will work with you and your child's provider to ensure he/she is caught up.

CHIRP (Children & Hoosier Immunization Registry Program) is a secure web-based application that is administered by the Indiana State Health Department and provides a database for student immunizations. ISBVI can use this registry to review vaccination records, therefore, please ensure your child's most recent vaccinations have been uploaded to CHIRP by his/her provider.

In the case of medical and religious exemption, a form is provided below.

If you have further questions about immunizations, please review the Indiana State Department of Health website (https://www.in.gov/isdh/17094.htm) and the Center for Disease Control (CDC) website (https://www.cdc.gov/vaccines/schedules/index.html)
